Housing Counseling Program
This program provides HUD certified counseling services to Monmouth County resident that may need housing counseling to prevent housing foreclosure.
Housing Counselors work with homeowners to avoid foreclosure in Monmouth County. Counselors offer information on the foreclosure process, and can provide one-on-one assistance to secure community resources, develop a household budget, and negotiate with lenders for repayment strategies. Housing Counselors can give you advice on buying a home, renting, defaults, foreclosures, credit issues and reverse mortgages.

The New Jersey Housing and Mortgage Finance Agency has developed the Mortgage Assistance Pilot (MAP) program to provide temporary financial assistance to income-eligible homeowners who wish to remain in their homes but are in imminent danger of foreclosure due to short-term financial problems beyond their control.
Case Management Services
This is a comprehensive case management program intended to help low-income individuals and families achieve self-sufficiency. Case management Assistance is a key component of every service we provide. The case manager will assess client problems and develop a case management plan which will assist the client in reaching the goal of self-sufficiency.
Case management facilitates the achievement of client self sufficiency through advocacy, assessment, planning, communication, education, resource management, and service facilitation. Based on the needs and values of the client, and in collaboration with all service providers, the case manager will write goals and objectives using expected or targeted outcome(s) and indicator(s). Quantifiable outcomes and outcome indicators will be identified for each planned service or intervention. The case manager then links clients with appropriate providers and resources throughout the continuum of health and human services, while ensuring that the care provided is safe, effective, client-centered, timely, efficient, and equitable.
Case management will be facilitated through and managed by ETO (Efforts to Outcomes) computer software by Social Solutions. This software allows caseworkers to better see the results of their efforts and change tactics based on real time, easily accessible data. This software will be used agency-wide to link all client case management activities.
The objective of the program is To provide information to participants that will enable them to better utilize services in their community including health, social, employment and other beneficial services. Case management provides advocacy, communication, resource management and promotes cost-effective interventions and outcomes.
